Please provide the full name, address, and phone number of your current and any other veterinarian(s) that you have used the last 5 years. IMPORTANT: After completing this application you need to call your vet and let them know you are applying for a rescue dog and give permission for information to be released to American Lab Rescue about your current and/or past pets health status. Without permission from your vet, we will immediately discontinue the process of moving forward with your application.
